Depending on the type of shipment, the material below will be useful for filling out the claim form and for uploading in support of your claim:

      • House Bill of lading

      • Master Bill of lading

      • Commercial Invoices

      • Packing List

      • Shipping instructions

      • Destruction/Salvage Certificates

      • Inspection Reports

      • Clear color pictures of the damaged areas claimed

      • Invoices – completed repairs or self-repaired

      • Any other documentation which will assist in evaluating your claim
